CONSENTING PROCESS
Print out the following document:
Most Updated Consent Form – To be signed by participant and by Raj
PET Facility Information Form – To be filled out by participant and then filled out by PET center
Documentation of the Informed Consent Process – To be filled out ONLY by Raj once participant consented (physical signature)
SECURING PET SLOTS
Once all documents have been filled out or signed, scan all documents and send electronic copies to James. The subject line should say the following: [SECURE: PET Consent - Luna Group].
It is important to write in SECURE in the subject line to lock the email since you will be sending patient/subject confidential information
It is crucial to submit these forms 5 business days (not including weekends!) prior to the desired slot. However, the earlier, the better!
Pass of to Lead RA in charge of PET study so they can file away in Luna Lab.
THE DAY BEFORE THE VISIT:
Send reminder email day prior to visit and get a final confirmation of their attendance (LOCATED IN SPA7T/PET folder) Advise sub to come in at least 15 minutes prior to their scheduled time.
WHAT TO KNOW (have it noted somewhere before going up to PET Center):
THE DAY OF THE VISIT:
No materials to take to the PET Center – just something to keep you entertained!
Meet sub at the Presby lobby entrance and walk up to the 9th floor PET Center. Let James know that you have arrived. The subject will then be weighted by James.
[Female] – head to Mara and Michael’s office to get a pregnancy test and guide sub to the Patient Only restroom to urinate in the cup. Once done. Take urine to the backroom to apply for a pregnancy test. Grab a plastic pipette – sometimes it's in the test packaging itself but if not they will be on the drawer to the right.Grab a small sample of urine with the pipette and add about 3 drops to the little well/hole designated by the test. Wait 2-3 minutes for results. Then, document on the packaging of the test. Write the following: Date, Time, RA initials, and test results (Pos. or Neg.)
Sub will be directed to an Exam Room to relax and get prepped for IV. The subject will have to wait a bit (with the IV already in place) before tracer is administered.

Select “Create Custom Dashboard”
Dashboard title: PET
Select Instruments/Events: Deselect all events except for MR MRScanLog, MR_2 MRScanLog, MR_3 MRScanLog, and PET
Filter Logic: [record_id]>10000 and datediff([behavioral_arm_1][demo_dob],'today',“y”) >= 18
This logic filters out all non-enrolled Luna ID's and only shows participants >=18 years old
